The Tabacalera Civic Centre will host the CIRCE Cantabria Conference, “Preventing Frailty from Primary and Community Care: Lessons Learned and Future Challenges”, a meeting that will bring together healthcare professionals, institutions, patients, and community agents with the aim of sharing results, experiences, and challenges in the prevention of frailty among older adults.
The event, which will take place on 4 February from 16:00 to 19:30 with free prior registration, is part of the European CIRCE Joint Action, an initiative funded by the EU4Health programme that positions Primary Care as a strategic pillar to strengthen the resilience, accessibility, and sustainability of health systems across the European Union. Its goal is to enhance Member States’ capacity to transfer and implement evidence‑based best practices, promoting innovative care models from the first level of care.
CIRCE is a joint action led by the Andalusian Health Service, involving 49 organisations from 14 Member States, including the Valdecilla Health Research Institute (IDIVAL) and the Cantabrian Health Service (SCS). Within this framework, Cantabria is replicating in its Primary Care system the best practice of Health Promotion Centres (HPCs), originally developed in Slovenia, under the leadership of the SCS Primary Care team.
The programme will begin with an institutional opening followed by the inaugural lecture, “The CIRCE Project: One Year Later. Results and Lessons Learned in Cantabria”, which will provide an overview of the project’s first year of implementation in the region, its alignment with the European context, and its impact on professional practice, participants, and the community environment.
Two dialogue sessions will follow, focusing on practical experience and future challenges. The first will be a roundtable with Primary Care professionals and participating patients, aimed at sharing first‑hand experiences. The second will be a challenges roundtable addressing key factors such as nutrition, polypharmacy, and social frailty, broadening the perspective towards a comprehensive, preventive, and person‑centred approach.
The event will conclude with a community synergies session designed to reflect on the role of community action in preventing frailty, followed by a closing session summarising the main insights and reinforcing the alliance between the health system and the community.
With this event, Cantabria strengthens its commitment to integrated, preventive, and evidence‑based care models aligned with the objectives of the CIRCE Joint Action, and consolidates the role of Primary Care and the community as essential pillars for addressing the challenges of ageing and frailty through an innovative and collaborative approach.
